Before you return: gather your follow-up documents in one file

Before leaving, ask the treating team which documents you'll need for follow-up, who to contact after you return, and how to send any later questions.

Check that you have the discharge summary, the required reports, prescriptions and written instructions, and make sure you know when and how follow-up will happen. Keep your invoices and any insurance or reimbursement documents you need.

Ask the doctor whether your travel date is suitable and about any special arrangements for the trip, and don't rely on another member's experience to set your date or change your medicines.

For administrative coordination, contact Sila from "My requests". Questions about symptoms or treatment should go to the medical team; replies in the community are not an emergency channel.
